| The health and safety of your child is very important to us at LHBC. That is why we request for each parent to fill out a Child Information Sheet for their child. If you have yet to fill one out, please take the time to do so now. This insures the quality of programming and safety that we can provide for everyone involved. You may also pick up a copy of the form from the church office at any time. We want to work with you to help your child grow to be everything that he or she can be.
